We do not request detailed diagnoses, medical records, Social Security numbers, insurance member IDs, or payment information through these forms. We do not collect unnecessary health information.

We use submitted information to understand interest in services, review eligibility and availability, consider professional opportunities, and respond to partnership inquiries. Joining the waitlist or submitting a profile does not establish a patient-provider relationship, employment, a contractor relationship, or a partnership.
Document Uploads
Documents uploaded by professionals are stored privately and are never made publicly accessible. They are reviewed only by authorized administrators in connection with consideration.
Data Sharing
We do not sell your information. We use trusted service providers to store and process data securely. We disclose information where required by law or to protect safety.
Your Choices
You can request access to, correction of, or deletion of your submitted information by contacting us. You can decline to submit any form. Optional marketing consent is separate from required contact consent and is never preselected.
